Adhering to a medication schedule can be a daunting task for many individuals, particularly those managing chronic conditions. Our community recently shared their experiences with the challenges they face in sticking to their medication regimens, highlighting several key barriers that hinder adherence.


Specialty Pharmacies: A Major Obstacle

A recurring theme in the responses was the difficulty posed by specialty pharmacies. These pharmacies, which handle complex and often high-cost medications, were cited as a primary reason for missed doses. One community member expressed frustration with Accredo, a specialty pharmacy benefit manager, citing numerous delayed shipments due to various excuses. They noted that the automated and human systems in place were unhelpful when trying to troubleshoot issues, resulting in missed scheduled doses. This individual emphasized that pharmacy benefit managers (PBMs) like Accredo seem more focused on cost savings than on supporting patients in managing their diseases.

Another community member mentioned difficulties with specialty pharmacies failing to ship medications using proper protocols. This led to delays and the delivery of unsafe medications. They also highlighted the challenges posed by delays in obtaining insurance authorization.


Insurance Companies: An Additional Hurdle

Insurance companies were identified as a close second to specialty pharmacies in terms of the barriers they present. One community member noted that pharmacies often state that insurance will not cover a medication until a certain date. Despite attempting to fill prescriptions at least five days in advance, they still encounter issues. This lack of clarity and timely coverage from insurance companies can disrupt medication schedules and lead to missed doses.


Impact on Daily Life

The challenges in adhering to medication schedules can have a significant impact on daily life. A community member shared that their medication, specifically prednisolone, has caused excruciating and unpredictable leg cramps. These side effects have made them feel unsafe to drive. Despite leaving messages for their consultant, they have received no calls back and feel unsure of where to turn. This example highlights how medication adherence issues can affect not only a person's health but also their overall quality of life.


Forgetfulness and Brain Fog

While systemic issues with specialty pharmacies and insurance companies are significant barriers, individual factors also play a role. Several community members mentioned that forgetfulness and brain fog are common reasons for missing doses. Despite setting reminders, they often dismiss them, intending to take their medication later, only to forget within minutes. This highlights the need for strategies to combat forgetfulness, such as pill boxes or alarms, to support medication adherence.


Conclusion

Adhering to a medication schedule can be fraught with challenges. From the systemic issues posed by specialty pharmacies and insurance companies to individual factors like forgetfulness, numerous barriers can hinder a person's ability to stick to their regimen. It is essential to address these challenges proactively, through improved communication, better support systems, and the implementation of reminder strategies. By doing so, we can work towards enhancing medication adherence and ultimately, improving health outcomes.
